A content item that is a marked-content sequence: an integer MCID in a structure element's K, or a marked-content reference
dictionary (/Type /MCR).

ISO 32000-2 §14.7.5.2, Table 357. The sequence is the one in a content stream whose property list has this Mcid:
the stream is ContentStream (Stm, a form XObject or an annotation appearance) when the MCR names one,
otherwise the content of Page. MCIDs are unique within one content stream, not within a page (§14.7.5.2).
The page is the MCR's Pg, else the element's Pg (Table 357: the MCR's overrides). When neither has one, the
nearest ancestor element's Pg is used as a repair, else the page whose parent-tree array lists the element at this MCID, with a
StructElemPageMissing diagnostic.
