A structure element: one node of a document's structure tree, a live view over its structure element dictionary.

K on each call. Two views of the same dictionary are equal.
The tree's shape comes from K, read downward from the root: Parent is the element whose K holds this one,
not what this element's P entry says. A P that disagrees is a StructElemParentMismatch diagnostic. A child
that is the element itself or one of its ancestors (a cycle) is left out of Children with a StructTreeCycle
diagnostic, and the hierarchy is cut below 256 levels (StructTreeDepthExceeded), so a recursive walk over
Children always ends.
